i#3391: dynamically identify APC CONTEXT offset

Merged Derek Bruening requested to merge i3391-win10-1809-APC into master

Adds code to look for the OP_lea referencing the CONTEXT in KiUserApcDispatcher rather than hardcoding a constant, as the offset has changed on Win10-1809.

Tested on Win10-1809, Win10-1803, and Win8.1.
